Tell a friend, family member, or anyone who needs a great eye doctor. Make sure they write your name on their new patient registration paperwork.

- Participants must be 18 or older.
- Your referral must write your name on their new patient registration paperwork — that's how we track it.
- Referrals count once the new patient completes their first appointment or service.

The referral program is open to current patients of Look + See Vision Care aged 18 and older. Effective March 1, 2024, while supplies last. A qualifying referral is a new patient who completes a purchase or service and mentions the referrer’s name at registration. Referrers receive a t-shirt after 3 qualifying referrals. Rewards are distributed in person only and subject to verification. Look + See Vision Care reserves the right to substitute rewards of equal or greater value, disqualify participants found in violation of these terms, or modify or terminate the program at any time. Fraudulent referrals will result in disqualification. Participation constitutes acceptance of these terms.
