Routine eye exams
Many EyeMed plans include a routine eye exam benefit with a copay. This may apply to a comprehensive eye exam, refraction, and routine vision evaluation.
Your exact eye exam copay depends on your specific EyeMed plan.
If your visit is medical in nature, for example red eye, eye pain, dry eye treatment, eye allergies, inflammation, glaucoma monitoring, diabetic eye care, or another medical eye condition, your medical insurance may apply instead of your vision plan.

Prescription glasses
Many EyeMed plans include benefits for prescription glasses, but the details depend on your plan.
Your EyeMed glasses benefits may include:

Book your appointment. Schedule your eye exam online or call our office. Let us know you have EyeMed when you book.
Send or bring your insurance information. We’ll need the subscriber’s name, date of birth, and any member information you have available.
We verify your EyeMed benefits. We’ll review your specific EyeMed plan and confirm whether your plan can be used at LOOK + SEE. We’ll help explain your exam copay, frame allowance, lens benefits, contact lens options, and any plan limitations.
You choose how to use your benefits. If your plan includes material coverage, we’ll help you decide whether to apply it toward prescription glasses or contact lenses.
We bill EyeMed directly when eligible. If your EyeMed plan can be used at LOOK + SEE, we bill EyeMed directly for eligible benefits. You are responsible for copays, non-covered services, lens upgrades, frame or lens costs above your allowance, contact lens fitting fees, and any services not included in your plan.
No mail-in reimbursement forms when your plan is eligible.
